1/15/12 –        Second Payment Due (15% of cost -m $502.50)

3/1/12 –          Third Payment Due (20% of cost – $670)

3/27/12 –        Forms Distributed to Participants

4/15/12 –         Fourth Payment Due (25% of cost – $837.50)

4/17/12 –         All Forms Due

5/25/12 –         Final Payment of Remaining Balance due ($1,140)

6/25/12 –         Departure from Newark/JFK International Airport
